After you determine your eligibility you'll need to submit the CUSP application which can be downloaded here. Please take the time to completely fill out the application. After we receive your completed application, it will be reviewed by a committee of utility industry peers/CUSP's. You will be notified by email within 10 business days if your application has been approved.
Once your application is approved, go to the CUSP Dates & Locations page and locate the date and location that you want to attend. Register to attend the optional Utility Safety Leadership Review and CUSP exam. Do not register until your application has been approved.
CUSP candidates are required to disclose any conviction of a felony or misdemeanor or having a professional license or certification revoked. If you checked “yes” on any of the boxes in the attestation statement on the application, you must complete the Ethical Conduct Questionnaire and send it with your CUSP application.
The Ethical Conduct Questionnaire will be reviewed by the Ethics Committee. Complete the Ethical Conduct Questionnaire form, then sign and fax to 815.425.6305 or email the Ethics Committee.
The CUSP program does not discriminate among candidates on the basis of age, gender, race, color, religion, national origin, disability, marital status, sexual orientation or military status.

The CUSP certification employs a rigorous process to ensure the accuracy of scoring the CUSP exam. The exams are scored electronically and the opportunity for error is highly unlikely. If you have a dispute of any kind, please complete the CUSP Appeals Form below within 30 days of receiving your results.
